1. Do lekarza należy zabrać:

  • Dokument potwierdzający tożsamość pacjenta [dowód osobisty, paszport, legitymacja szkolna lub studencka],
  • Numer PESEL pacjenta [dotyczy wszystkich pacjentów powyżej ukończonego 3-miesiąca życia],
  • W przypadku pacjentów zgłoszonych do ubezpieczenia przez placówki mundurowe [wojsko, policja, straż graniczna itp.] warto zabrać dokument potwierdzający ubezpieczenie,
  • W przypadku ubezpieczenia przez instytucję MOPS, GOPS – aktualną decyzję,
  • W przypadku pacjentów posługujących się kartą EKUZ warto zabrać ją z dokumentem stwierdzającym tożsamość,
  • W przypadku chorób przewlekłych opakowania zażywanych leków.

  • Patients with chronic diseases are advised to bring original packaging of their medications.

Wenn Sie zum Arzt gehen, ist folgendes mitzunehmen:

  • Bei Patienten, die bei Polizei, Armee oder Grenzschutz arbeiten, ist es empfehlenswert, einen Versicherungsausweis mitzunehmen,
  • Wenn Sie  die E-Card verfügen, nehmen Sie sie auch mit,
  • Bei langwierigen Krankheiten Verpackungen von den Arzneimitteln, die eingenommen werden
